Letter Request Form
On request, the Department can provide enrollment and appointment verification letters for students and employees. The letters are provided with information as requested; however, we do not guarantee acceptance of the letters by the government or other authorities. All letters will be addressed “To Whom It May Concern” unless otherwise specified. Please allow for a minimum of three business days for request processing and note that requests are processed in the order received. An email will be sent when the letter is available for pick-up.
